QUIZ No. 32
CASE 32
A 24-year-old female with no known comorbidities presented with knee pain. No history of trauma. MRI knee was done. What’s your diagnosis?
MRI
MRI
Visitor No:
ANS – Anterior Suprapatellar fat pad impingement syndrome
A/K/A Quadriceps fat pad syndrome
Findings: Increased signal intensity of the suprapatellar fat pad. The anterior suprapatellar fat pad is edematous compared to the prefemoral fat pad, enlarged (≥10 mm AP diameter). It shows a convex posterior border with a mass effect on the adjacent suprapatellar recess.
Triangular fat pad posterior to the quadriceps tendon, anterior to the suprapatellar recess, and superior to the patella.
The anterior suprapatellar fat pad is an intracapsular, but extrasynovial structure.
Repetitive friction or compression between quadriceps tendon and femur → inflammation, edema, and enlargement of the fat pad.
Management - Conservative: Rest, NSAIDs, physiotherapy focusing on quadriceps and VMO strengthening.
Take home points
Quadriceps fat pad edema with convex posterior border compressing suprapatellar recess is a hallmark imaging sign of quadriceps fat pad impingement.
